Organization Funds Request

Please follow the steps below to request funds for an organization
Requests will be reviewed at our monthly Business Meeting, which is held the 1st Tuesday of each month.

Submission Deadline: 5 days prior to Business Meeting.
  • Fill out this form as much as possible.
  • Click the SUBMIT button on the bottom of this page.
  • After reviewing your request, we may contact you to attend one of our meetings and present your request to our club.
  • A financial statement from your organization may be requested.
  • All 501(c)(3) & 501(c)(4) organizations must submit an IRS tax status determination letter, or have one on file with the Clear Lake Lions.
  • Request may take up to 90 days for a response.

    If you answered YES to the question above, please upload your organization IRS determination letter below.
    Please note: your organization's legal structure and tax status must be submitted prior to your request being considered.

    If your organization is a non-profit/501c3, please upload IRS determination letter.
