To request donations from the Clear Lake Lions please follow these steps.
- If you are an Organization requesting funds click here.
- If you are an Individual requesting funds click here.
- Save or Open the file.
- Fill out the form.
- Email the form to requests@clearlakelionsmn.org
Or mail the form to the address on the bottom of this page.
Additional Information
- Requests must be received prior to the Board meeting in which you are presenting.
- Board meetings are held on the first Tuesday of the month.
- Board meeting dates, times, and location are on the Calendar.
